4-Year Old Kindergarten

Registration Information 2013-2014

Registration is OPEN for the 2013-2014 school year.  You may register your child at any of the 4K sites during normal business hours. You may also register your child at Wilson School between the hours of:  7:45 a.m. - 3:45 p.m.

Required Materials for Registration

  • Birth Certificate (original)
  • Immunization Record
  • Phone/Address information of doctor and emergency contacts
  • $30 Registration Fee
  • Show proof of residency.  (e.g. - copy of electric or heat bill, rental agreement)

Site assignments will be based on need for child care services, place of residence, special programming needs, transportation and available openings at the site requested. The Beaver Dam Unified School District will determine all final placement decisions.
