Welcome to the 2012 Beaver Dam Community Hospitals, Inc. Nurses 5k Challenge

Saturday, May 19th, 2012 (rain or shine!)

Packet pickup/registration table open at 7:00 a.m. Race starts at 8:00 a.m.

Proceeds from this event will be directed to the

Beaver Dam Community Hospitals Foundation, Inc. Health Care Educational Fund.

Entry Fee:

Pre-registration (received by May 1)… $20.00

Day of Event registration...............….$25.00

Children (under age 15) ………..….$15.00

Cash or check only – make checks out to Beaver Dam Community Hospitals, Inc. (*Registration fees are nonrefundable; if you are unable to attend, we will mail your t-shirt to you.)

We have no age limit, but we do ask for no bikes, roller blades, "heelies," skateboards or dogs. Thank you for your understanding.

Your entrance fee guarantees you a t-shirt that is specially designed for the 2012 race. T-shirts for day-of-race registrants will be distributed on a first-come, first-served basis. If you do not receive yours on the day of the race, we will mail one out to you within three weeks of the race date.
