4 Year-Old Kindergarten Information

Enrollment Information

Enrollment for 4-year-old Kindergarten can be completed at the Educational Service Center located at 400 E. Burnett Street, Beaver Dam.

Please bring the following information for enrollment:

• Completed Enrollment Forms (click on the link on the right-side of this page) 
• Original Birth Certificate
• Current Immunization Record
• Proof of Residency (see list below for required documents)
• $30 Materials Fee (cash or check only)

Proof of Residency Requirements

TWO (2) documents are required to prove residency (one (1) from each category)

4K Site Assignments

4 Year-Old Kindergarten Site Assignments:

All district resident children who are eligible for 4-year-old kindergarten during the 2025-26 school year who currently attend a child care center in which 4-year-old kindergarten classes occur, qualify for Legacy Registration. This means they are guaranteed placement in that child care center if the parent chooses to enroll them there. AM/PM sessions are not guaranteed. Legacy registration must occur on or before March 14, 2026, for guaranteed placement.

Non-Legacy Registration placements will be based on the order registrations are received, specialized needs of the students, the need for child care, and available openings at the site requested.

The Beaver Dam Unified School District will determine all 4-year-old kindergarten site placements.
